To create your own form (questionnaire) for your own purposes go to the google docs site.

Review this instructional video about Google Docs……it's a great resource tool! Don't forget to take a few moments to fill out the Teacher Transition Questionnaire (a great way to see what can be done with this great application). This would be a great information gathering tool to use with parents,teachers, or students.

Have you filled out the teacher questionnaire yet? It came from doc.google.com and is very cool way to gather information and easy to use. Click on this teacher questionnaire link (if you've not already) and take a moment to fill it out, the answers are tabulated automatically and the final results will be distributed to you all!
